Information we collect

Ascend Rx Performance may collect information you provide through the private member portal, including your name, email address, telephone number, account information, service requests, scheduling details, support communications, and information you voluntarily submit through forms.

Adult membership

Applications are limited to people 18 years of age or older. We use your self-reported birth date to check eligibility and retain the result and time of that check, not the birth date itself. This is not independent identity verification.

How we use information

We use information to operate and secure the portal, administer memberships, respond to requests, coordinate appointments, provide account support, maintain records, and send communications you have requested or consented to receive.

SMS communications

If you expressly opt in, we may send non-marketing text messages concerning membership applications and account updates, appointment confirmations and scheduling changes, coaching or consulting check-ins, required-document reminders, and account support, where relevant to you. Marketing and shop/order messages are excluded. Consent is optional and is not a condition of membership or purchase. Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for assistance.

We record the mobile number you consented to use, the date and time of consent, the consent wording and disclosure, its version, and the form used to provide consent. Providing a telephone number alone does not constitute consent.

Mobile information, including telephone numbers and SMS consent records, is not sold, rented, or shared with third parties or affiliates for their marketing or promotional purposes. We may disclose information to service providers only as necessary to deliver requested communications, operate the portal, comply with law, or protect rights and security. SMS consent is not shared with third parties for purposes unrelated to providing the messaging service.

Service providers and security

We use service providers to support authentication, hosting, data storage, email, and text messaging. We take reasonable administrative and technical measures to safeguard information, but no method of transmission or storage is completely secure.

Your choices

You may opt out of text messages at any time by replying STOP. You may request assistance by replying HELP. You may also contact us to request access to, correction of, or deletion of personal information, subject to legal and operational recordkeeping requirements.
